New Early Childhood Students

New student enrollment for Early Childhood begins at the Madison Early Childhood Education Center at 130 W. Madison St. Please call (630) 617-2385 to make an appointment. 

 

Current D205 Families

Needing to Provide Residency Verification

Please contact your school secretary for the best time to bring your re-verification of residency documents (e.g. expired or soon to expire lease; special residency situation) through the end of the school year.

District Boundary Map(click to view): This new tool will help parents and realtors better determine which elementary and middle school boundaries a particular address falls in. All students attend York High School for their freshman through senior years. Please note that this tool is for informational purposes only. You should check with the District Office at (630) 834-4530 to confirm your boundaries.

NEW STUDENT ENROLLMENT CHECKLIST

Parent/Guardian must submit ALL of the following documents. Please contact the registration staff if you are having trouble collecting required documents.

  • Photo ID – Driver’s License, Photo ID or Foreign Consulate ID.
  • Student Original Birth Certificate with embossed seal.
  • Legal Custody documents (if applicable)
  • Three proofs of residency
    • A total of three proofs of residency, outlined below, are required for all new students, incoming 9th graders, and parents with expiring verifications. Please contact the school if you are having trouble collecting the required documents.

      Documents must be physically presented to school staff. If you have questions about when the best time to visit the school you can visit the school website for school office hours or call the office to arrange a time to bring in the required paperwork.


One of the following for the residence at which the student(s) reside(s):
  • A current real estate bill showing parent/guardian as the owner 
  • A current original signed lease showing the parent/guardian as the tenant 
  • A current mortgage statement showing the parent/guardian as the owner

AND

Two of the following showing the address for the residence at which the student(s) reside:

Please note the following: 

  • If original documents are presented, copies of these documents will be made on the spot and the originals returned to you. 
  • Partial documentation will not be accepted. All documents must be presented in order to be processed.

  • A copy of each student’s physical form for early childhood, kindergarten for elementary school, 6th grade for middle school or 9th grade for high school.
  • A copy of each student’s vision exam form for all new student.
  • A copy of each student’s dental exam form for students entering kindergarten, 2nd grade and 6th grade of the current school year.
  • A copy of each student’s immunization record.
  • A copy of each student’s most recent IEP or 504 plan and evaluation report (if applicable).
  • Transportation registration form, applicable to special education students who qualify for bus service.
  • Student’s world language experience or preference for grades 6-8.
  • Student’s interest and experience with band, orchestra or chorus for grades 6-8.
  • A copy of the student’s classes and grades from previous middle or high school.
  • If you have record of the student’s standardized test scores, please provide.
  • Special Residency Affidavit English 
  • Special Residency Affidavit - Spanish
  • Residency Affidavit for Divorced or Separated Families
  • If a custody agreement exists for the student, this must be provided to the child’s current school if a returning student or the school in which the child is enrolling for a new student to the district.

For mid-term enrollment, please contact your school for a pro-rated fee total. When you have completed the online data entry and submitted payment of school fees, the registration process is complete.
